Certificate Information

· The certificate issued from this order form is for additional recognition of international certification and display purposes only. It should not be used by an IFSAC accredited entity as a certification certificate.

· All certification information (Entity, Discipline/Level, Date of Certification, and Seal Number) on the front of this form must be completed in order for an IFSAC certificate to be issued.

· A certificate will only be issued to individuals who are currently registered as certified by an IFSAC accredited entity and have an IFSAC seal number(s) for each level of certification. Upon verification of the certification information on the order form, an IFSAC certificate will be prepared and mailed.

· This certificate does not contain the original certification seal number and should not be used to verify an individual’s certification levels.

Certificate Ordering Information

· All orders must be prepaid at $15.00 per certificate.

· Only one level of certification per certificate (i.e. Fire Fighter I).

· Payment can be made by check, money order, or credit card.

· Make checks and money orders payable to the International Fire Service Accreditation Congress, or IFSAC.

· If using your MasterCard, Visa, American Express, or Discover Card, complete the credit card information on the order form.

· Oklahoma residents add 8% sales tax before shipping and handling.

· Orders are normally processed within 48 hours of receipt. Shipping takes 10 working days under normal circumstances; however, we ask that you allow 4 weeks for delivery. If you do not receive your order within 4 weeks, call IFSAC Administration at 1-405-744-8303.

· Return Policy: IFSAC guarantees this certificate and frame. If you are not fully satisfied, you may return your purchase within 30 days for a full refund or exchange. If you have any problems or questions regarding your order, call IFSAC Administration at 1-405-744-8303.

· All orders are subject to acceptance by the International Fire Service accreditation Congress, Oklahoma State University, Stillwater, OK.
